Registered Nurse

The PRIMARY NURSE CLINICIAN is responsible implementing and maintaining the patient’s plan of care; and monitoring, accessing and evaluating the effectiveness of the care, treatments and services being provided to meet the patient’s needs. The Primary Nurse Clinician coordinates the patient’s direct care, and communicates progress toward goals and/or any changes needed for the plan of care with the supervisor.

Job Requirements
1. A Registered Nurse with two years of clinical experience within the last three years in a home health agency.2. Must meet educational/experience requirements outlined in the laws of licensure states.
3. Must possess knowledge of Medicare and Medicaid requirements outlined in the laws of licensure states.
4. Excellent organizational, interpersonal, telephone, communication and documentation skills.
5. Ability to relate to and treat acutely ill/terminally ill patients/families and their particular problems.
6. Must possess strong clinical assessment, evaluation and patient education skills.
