Physical Therapist

The PHYSICAL THERAPIST is a qualified health care professional who directs, supervises, evaluates, and provides physical therapy services to clients in the home as prescribed by the attending physician. He/she is responsible for: (1) providing skilled physical therapy care to patients with impairments, functional limitations, disabilities, or changes in physical function resulting from injury, disease, or other causes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned. He/she works under the supervision of the designated supervisor

Job Requirements


1. Graduation from a physical therapy school accredited by the Commission on Accreditation in Physical Therapy Education.

2. Licensed or registered as a Therapist in the state(s) in which he/she is required to practice.

3. A minimum of one year of experience in acute, rehab, or long term care setting required; previous home care experience preferred.

4. Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.

5. Must possess strong patient assessment, patient management, and documentation skills.

6. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.
