CLIENT OPT-OUT REQUEST
By entering your name and e-mail address, you will be able to remove your client application from the Katrina's Angels system before you are processed by a volunteer. If a volunteer has already contacted you about your application and has taken your information, this opt-out form will not take you out of the system. In this case, you will need to contact the Guardian Angel assigned to you to request withdrawal from the Katrina's Angels Client Case Management System.
